Clinical Pharmacy Review

As part of our Integrated Family Health Centre model of care, the general practice teams at Hamilton’s three NorthCare sites had been extended to include the clinical pharmacist (CP) role. 

This role is designed to support the practice team, help patients to manage and monitor their medications and care plans, and liaise with local and hospital pharmacies to make sure that medicine-related information is shared seamlessly. Nine months on, a review of the role looks at the types of activities undertaken by the CP and what effect this work has had on general practitioners and practice nurses.
