Primary options acute care

Primary options acute care is a programme designed to improve health outcomes and patient experience. The programme funds primary care to provide safe, acute care within the community. This reduces demand on acute hospital services by preventing unnecessary referrals to the emergency department and consequently hospital admissions.

Patients may be considered for services where, in the absence of primary options for acute care (POAC) funding, the patient would be referred to hospital.
